When notifying the NRC, the person reporting the incident must provide the following information:
- Name of reporter;
- Name and address of person represented by reporter;
- Telephone number where the reporter can be reached;
- Date, time, and location of the incident;
- Extent of any injuries;
- Class or division, PSN, and quantity of hazardous material involved in the incident, if available; and
- Type of incident and nature of
hazardous materials involvement
(e.g., spill, explosion) and whether
continuing danger to life exists at the scene of the incident.
