Some incidents require that notice by telephone be provided to the NRC or CDC within 12 hours of the incident, followed by a written report to the DOT Pipeline and Hazardous Materials Safety Administration (PHMSA) within 30 days.
Other hazardous materials incidents require only a written report to PHMSA within 30 days of the incident.
Each report must include the information identified in § 171.15 and/or § 171.16. A follow-up report may be required.
